Our schools depend upon our parent community and volunteers to make it the exceptional district it is, year after year. Thank you! To volunteer at any OUSD school, parents & guardians need to complete the following clearances for the volunteer role:

Volunteer Roles:

School-site Volunteers (Classroom, Hot Lunch, Library, Art, P.E., etc.)

Clearance Types:

TB Clearance

Valid for 4 years
If you plan to volunteer in ANY capacity, you must show you are free of communicable tuberculosis (TB). The Contra Costa Health Department REQUIRES all volunteers, whether in the classroom, on field trips, or in the hot lunch program, to provide proof of completing a TB assessment/screening. 
To comply, you must bring a completed Adult Tuberculosis Risk Assessment Form to a registered health care provider and submit a copy of the Certificate of Completion page to Sleepy Hollow. If you are unsure if you are current, please contact the front office (925-254-8711).

--> If you plan to volunteer at multiple schools, please make a copy of your form and send it to each school.

Fingerprint Background Check Clearance

Valid through 8th grade in OUSD schools 
Parents and guardians driving students and/or chaperoning overnight field trips must be fingerprinted for criminal background by the DOJ/FBI.  Unfortunately, fingerprints are not transferable from another organization. 
The criminal background check requirement shall not apply to volunteer supervisors for lunch or other nutritional periods or to volunteer as a non-teaching aide under the immediate supervision and direction of certificated personnel pursuant to Education Code 35021, including parents/guardians volunteering in a classroom or community members providing non-instructional services.
  • Search for a Live Scan location near you (the Orinda Police Department has the fastest turnaround)
  • Bring a completed Live Scan Fingerprint Form to the fingerprinting
  • Bring a government-issued photo ID to the fingerprinting
The results are sent electronically to the district office. The processing may take several weeks. 
 

Driver Liability Clearance

Valid for 1 school year, must be renewed each year
All field trip drivers must have Driver Liability Clearance to participate. 
  • Completed and signed Use of Private Vehicle Insurance Coverage Form
  • Copy of your valid Driver's License with your current address
  • Proof of automobile insurance coverage of at least the following amounts:
    (This is found on the "Declarations" page on your insurance statement, NOT the Insurance ID card)
    • Bodily injury $300,000
    • Property damage of $50,000
    • Or a combined single limit of $300,000
    • Medical payment of $5,000
